Coverage for Cancer and Associated Conditions Drug Treatment: Prohibits insurers of certain individual & group health insurance policies from requiring, before drug is covered, that insured fail or have failed to successfully respond to different drug; prohibits such insurers from excluding coverage for certain drugs on specified ground; requires coverage for specified services. Effective Date: January 1, 2021
