Health Insurance: Requires individual accident or health insurance policies, group, blanket, or franchise accident or health insurance policies, & managed care plans to evaluate & review coverage for orthotics & prosthetics & orthoses & prostheses; provides requirements & limitations; specifies deductible & copayment requirements; authorizes insurers to specify certain benefits limitations; provides for nonapplication to certain policy coverages. Effective Date: July 1, 2015
